Technical Assistance:
Discussed resources for additional training for staff. You are reminded that MAT training can not be counted towards the annual 16 hours of training.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 7 children (20pts) with Provider and 2 assistants were present today. When the inspector arrived, morning snack was finishing and children were playing freely in the large programming area. Hand washing procedures observed after snack. Staff transitioned the children to the smaller programming room and sang songs and provided ample toys and equipment for the children to play with. Songs were sung by staff and the children followed along. Observation of the bathroom, food prep area, and diapering was made. No medications on site today. All staff files and 6 children's files were reviewed. Completed artwork made today was on the drying rack. The children made painted artwork with stencils. Children transitioned to the bathroom and nap time after lunch. Staff observed sitting beside children on mats and encouraging nap time. Lunch today was pizza and fresh fruit. Standard #: 22VAC40-111-120-A
Description: Based on review of all staff records, one record was incomplete. Evidence: Staff #2's record contained only 1 of the 2 references required, the first day of employment and documentation of orientation training.

Plan of Correction: I will document all of this information and maintain in the record for this staff.

Standard #: 22VAC40-111-90-C
Description: Based on child record review, additional immunizations were not obtained every six months for 2 children. Evidence: Child #3 and #5 are both under the age of 2 years. Child #3's last documented immunizations was dated 8/6/18. Child #5's last documented immunizations are dated 6/6/18.

Plan of Correction: I will request updated immunizations from both of these parents and maintain them in the records.

Standard #: 22VAC40-111-240-A
Description: Based on observation of the crib room, one area was not maintained in a safe manner. Evidence: The large crib located on the left side of the wall has a mattress that is cracked in several areas on both sides of the mattress. This was a repeated issue during the licensing period.

Plan of Correction: I will dispose of this mattress and buy a new one. Until then, this crib will not be used.

Standard #: 63.2-1720.1-B-2
Description: Based on review of staff records, one background check was not obtained as required. Evidence: Staff #2's record has completed fingerprint background results that were obtained by another licensed Provider. Background fingerprint search results are not transferable.

Plan of Correction: This staff will complete new fingerprint background search and results will be maintained in the record.
