Comments:
An unannounced monitoring inspection was conducted today, August 23, 2019 to determine the family day home's compliance with licensing standards. The inspection was initiated at 2:15pm and concluded at approximately 4:00pm. In attendance were 6 children totaling 13 points being supervised by the provider. Children were observed playing with toys and interacting with the provider. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All rooms of the home used by children were inspected with no hazards found. Due to inclement weather during the inspection the outdoor play areas were not inspected. Administrative records were inspected including five children's records and the provider's records with the majority of the required information found. The following was also inspected: required postings, fire drill log, emergency preparedness plan, first aid kit & emergency supplies, parent agreements, injury reports, and other pertinent information. Standard #: 22VAC40-111-60-B
Description: Based on the inspection of children's records, the family day home did not ensure the required information was obtained. Evidence: During the inspection of five children's records, it was discovered child #3, #4, and #5 did not have a second emergency contact listed. Licensing standards require two emergency contacts to be listed with the full address and telephone number of each emergency contact.

Plan of Correction: Per provider: I will have the parents to give an additional emergency contact during pickup time.

Standard #: 22VAC40-111-90-C
Description: Based on the inspection of children's records, the family day home did not ensure updated immunizations were obtained. Evidence: During the inspection of five children's records, it was discovered that child #2 last documented immunizations were in 2016. Licensing standards require documentation of immunizations once every six months for children under the age of two years and once between each child's fourth and sixth birthdays.

Plan of Correction: Per provider: I will have the parent provide a copy of the child's most recent immunizations.

Standard #: 22VAC40-111-140-B-3
Description: Based on the inspection of administrative records, the required cardiopulmonary resuscitation (CPR) certification has expired. Evidence: During the inspection of the provider's record, it was discovered the cardiopulmonary resuscitation (CPR) certification obtained on 07/29/17, expired on 07/29/19 with no further updates in the record.

Plan of Correction: Per provider: I am scheduled to have my cardiopulmonary resuscitation (CPR) recertification on tomorrow 08/24/19.

Standard #: 22VAC40-111-140-B-4
Description: Based on the inspection of administrative records, the required first aid certification has expired. Evidence: During the inspection of the provider's record, it was discovered the first aid certification obtained on 07/29/17, expired on 07/29/19 with no further updates in the record.

Plan of Correction: Per provider: I am scheduled to have my first aid recertification on tomorrow 08/24/19.

Standard #: 22VAC40-111-210-A
Description: Based on the inspection of administrative records, the required annual training hours were not obtained. Evidence: During the inspection of the provider's record, it was discovered for the year 2018 the provider did not have the required 16 hours of annual training for the year.

Plan of Correction: Per provider: I will make sure I obtain the required 16 hours of training hours for 2019.
