54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Discussed that all prescription medications should be from a pharmacy and maintained in the original, labeled container. Discussed the hooks on the playground.

Comments:
An unannounced renewal inspection was conducted today between the hours of 9:30a through 11:30am with the provider on site. There were 12 children (39 points) with the provider and two assistants. A sample of 5 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, snack, playing in centers, dancing, listening to music, looking at books, bathroom breaks and hand washing. Infant's were observed playing in bouncy seats, tummy time, being held, bottle feeding, looking at books, diaper changes, hand washing and playing with toys on the floor. Interactions between the children and caregivers were positive. The physical space, emergency supplies, evacuation drills, attendance records, medications and the Emergency Preparedness Plan were reviewed. Description: Based on review, the provider did not review the emergency plan at least annually and update the plan, document in writing each review and update to the emergency plan. Evidence: 1. The provider's emergency plan was last reviewed on 6/11/2018 and needed to be reviewed by 6/11/2019.

Plan of Correction: I will review and update.

Standard #: 22VAC40-111-480-D
Description: Based on observation, equipment used by children was not maintained in accordance with the manufacturer's instructions. Evidence: 1. A blue infant swing on the playground equipment was missing the safety belt.

Plan of Correction: I will find a safey belt.

Standard #: 22VAC40-111-520-A
Description: Based on interview and observation, children birth through 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot were not provided a crib. Evidence: 1. The provider had 4 cribs in the home and had 5 children 12 months of age or younger. 2. The provider stated one child under 12 months of age was sleeping on a cot or mat.

Plan of Correction: I will purchase another crib.
