Comments:

An unannounced monitoring inspection was conducted today from 10:20am-11:50am. There were 5 children (15 points) directly supervised by the provider. The home, 1 staff record, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, medications and policies were inspected. Children were observed playing freely, listening to a story, playing with small manipulative toys, and eating snack. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. Areas of non-compliance are identified on the Violation Notice. Standard #:	22VAC40-111-80-A
Description:	Based on record review, the family day home did not have documentation that they had view the proof of identity of a child within seven days of the child's first day of attendance at the family day home. Evidence: Child#1's record does not have documentation that the provider verified the child's proof of identity within seven business days of the child's first day of attendance.
Plan of Correction:	I will ask the parents to bring back the birth certificate.
Standard #:	22VAC40-111-90-C
Description:	Based on record review, the family day home did not obtain documentation of additional immunizations once every six months for a child under the age of two years. Evidence: Child#3's record contain an immunization record that was last updated on 8/13/2018. The child was 12 months of age when the record was last updated.
Plan of Correction:	I have already requested the immunization records from the parents during the inspection.
Standard #:	22VAC40-111-180-A
Description:	Based on record review, the provider did not obtain a current report of tuberculosis (TB) screening every two years from the date of the last screening. Evidence: Staff#1's record contains TB screening/test documentation that was last updated on 8/3/2017. Household member#1's record contains TB screening/test documentation that was last updated on 8/3/2017.
Plan of Correction:	We will have the forms updated this weekend.
Standard #:	22VAC40-111-540-A
Description:	Based on observation, high chairs were not being used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened. Evidence: Three children were observed eating snack seating in high chairs without the safety straps fastened.
Plan of Correction:	I will make sure I always use the safety straps.
