Technical Assistance:
Technical assistance was offered regarding the outside play area, small group activities for the children. Provider had a question about increasing capacity and was offered guidance.

Comments:
An unannounced monitoring inspection was conducted from 10:15am to 12:00pm. There were 7 children (21 points) in care of two caregivers. The children were observed during circle time which included songs, letter recognition and learning about animal sounds. The children were using the blackboard, and observed cleaning up after playing with toys. The outdoor play area was assessed. There were no medications on site. The emergency drill records and plan, and first aid kit were reviewed. Areas of non compliance are listed in the violation notice. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not labeled.
Evidence: Two infant bottles were not labeled with the name and date. Two additional infant bottles were labeled with the child's name, but were not dated.

Plan of Correction: I will check all bottles to make sure they are labeled properly.

Standard #: 22VAC40-111-120-B
Description: Based on record review, the provider did not obtain two reference checks for an assistant caregiver.
Evidence: Caregiver # 1's record had documentation of only one reference check. The second reference check was not obtained.

Plan of Correction: I will obtain another reference for the caregiver.

Standard #: 22VAC40-111-60-B
Description: Based on records review, records for children in care were not complete.
Evidence:
1. Child # 1's record was missing the medical policy number.
2. The section for authorization for emergency medical care for Child # 2 and Child # 3's record was not completed by the parents.
3. Child # 3 was missing information for person authorized to pick up.

Plan of Correction: I will get the missing information from the parents.
