An unannounced renewal inspection was conducted on August 27, 2019 from approximately 10:20 am 12:40 pm. Provider was present with four children for a total of seven points. Four children's records were reviewed. Provider training was also reviewed.

Standard #: 22VAC40-111-920
Description: Based on observation, provider did not ensure when meals or snacks are provided there is a current week menu posted that is dated, posted or given to parents and lists foods served. Evidence: The menu posted was not dated and did not clearly list foods to be served.

Plan of Correction: Will update the menu today.

Standard #: 22VAC40-111-60-B
Description: Based on review, provider did not ensure 3 of 4 children's records have documentation of required information. Evidence: The following was found during record review: 1. The records for child #1 (start date August 12, 2019) and child #2 (start date August 12, 2019) did not have father's home address/phone number, father's employment information, mother's home address and first emergency contact address. 2. The record for child #4 (start date July 18, 2019) did not have father's employment information, physician phone number, insurance plan/number and first emergency contact address.

Plan of Correction: Will get the parents to give the information today.

Standard #: 22VAC40-111-280
Description: Based on review, provider did not ensure potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored in areas inaccessible to children. Evidence: Disinfectant spray and paint labeled "warnings...keep out of reach of children" were stored in an unlocked cabinet under the kitchen sink which is accessible to children.

Plan of Correction: Child lock on the cabinet will be kept secured.
