Comments:

An unannounced monitoring inspection was conducted on Thursday, August 1, 2019 to determine compliance with licensing standards. The inspector arrived at 2:05 PM and concluded the inspection at 3:25 PM. Today, there were three children, totaling six points, present with 1 caregiver. During the inspection, children were napping, eating snack, building with blocks and creating structures using popsicle sticks. Transportation is not provided. Children?s equipment, learning materials, indoor space and restrooms were inspected. Five children's records, one provider record, one household record, two pet records, daily attendance records, fire drill and shelter in place drill logs, and all required postings were reviewed. The provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Standard #:	22VAC40-111-180-A
Description:	Based on a review of caregiver records and interview, the provider did not ensure that subsequent tuberculosis (TB) screening for caregivers and household members was obtained. Evidence: 1. Caregiver #1 did not have documentation of a current TB screening. 2. Caregiver #1 stated that she had completed the TB screening and had a print out of the receipt. She stated she could not locate the documented results for the TB screening.

Description:	Based on a review of documentation, the provider did not ensure that household pets shall be vaccinated for rabies. Evidence: 1. The current shot record for dog #2 shows the rabies vaccination expired on 07/26/2019.
