Comments:
An unannounced monitoring inspection was conducted on Wednesday, August 28, 2019 from approximately 9:30 AM until 11:30 AM to determine the provider's compliance with licensing standards. There were 11 non-resident children and one resident child in attendance being cared for by two caregivers. Based on the children?s ages there was a total of 31 points. The children were observed playing with toys, watching children?s programs on T.V., singing songs, infants being held, rocked and fed and taking bathroom breaks and washing hands. During the inspection, the licensing inspector reviewed the following: emergency supplies, emergency preparedness plan, emergency drill logs, required postings, menu, caregivers? training hours and daily attendance. The provider stated medications are not being administered at this time. Transportation is not provided at this time. During the inspection five children's and two caregiver?s records were reviewed. Standard #: 22VAC40-111-240-B
Description: Based on observation, the provider used equipment, materials, or furnishings that was recalled or identified by the U.S. Consumer Product Safety Commission as being hazardous. Evidence: 1. On 8/28/2019, a Fisher-Price Rock 'n Play sleeper was observed in use by Child #4 at approximately 10:00 AM within the child care area of the Family Day Home (FDH). 2. All models of this product was recalled on April 12, 2019 by the U.S. Consumer Product Safety Commission. 3. The provider stated she was unaware of the recall for this item.

Plan of Correction: The Rock-N-Play sleeper was folded up during the inspection and the will be thrown away.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not ensure child-resistant protective covers larger than 1-1/4 inches in diameter were installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Evidence: The outlet located in the bathroom sink that was used by children under fiver years of age during the inspection did not contain a child-resistant protective cover.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-111-430-B
Description: Based on observation, the provider did not ensure pressure mounted gates were not used as protective barriers at a stair opening. Evidence: A pressure mounted gate was placed at the bottom of the stair opening located in the child care area that led into the kitchen. There were a total of 10 children in care in the child care area ranging from two months to six years of age.

Plan of Correction: I will have a gated that is not pressure mounted installed at the bottom of the stair opening.
