Comments:
The licensing inspector conducted an unannounced renewal inspection from 9:25am - 12:15pm. There were eleven children in care (one residential) for a total of twenty eight points with two caregivers. The children were observed playing with toys, watching cartoons and during lunch. Four caregiver's records, one household member's record and five children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Please notify the licensing office within 10 days with documentation of background check results or a copy of the request. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-111-60-B
Description: Based on record reviews, the provider failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #1) had not been updated annually by the child's parents and child's date of enrollment was 11/27/17. One of five records reviewed (child record #2) contained only one emergency contact.

Plan of Correction: The provider will request this information from the children's parents at pick up time today.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on record reviews, the provider failed to obtain a background for a new caregiver. Evidence: 1. The inspector reviewed four caregiver's records. One of four records reviewed (caregiver record #4) did not contain a search of the central registry and caregiver's date of employment was 05/01/2019.

Plan of Correction: The provider requested the background check and it was returned two times. The request has been resubmitted and as soon as the results are obtained the provider will send a copy to the licensing office. The provider contacted the Office of Background Investigations during the inspection and is waiting for a return call.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on record reviews, the provider failed to obtain background checks for a new household member within 30 days of the person beginning to reside in the home. Evidence: 1. The inspector reviewed one household member's record. Household member's record #1 did not contain a sworn statement nor a search of the central registry. Household member has been living in the home since 07/08/2019.

Plan of Correction: The provider will ask the household member to complete a sworn statement today. The provider will ask the household member to complete a request for a search of the central registry and submit on 08/31/2019. The provider will send a copy of both background checks to the licensing office when they are obtained.
