Technical Assistance:
We discussed the requirements for medication. The risk assessments for violations was provided.

We talked about making sure the sandbox is never accessible to feral cats.

Also, we talked about options for sanitizing solutions used on tables and the diaper mat.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 10:45 AM to 1:18 PM. Today, there were eight children, ages two through four years, in the care of the provider. I viewed program activities, daily routines, equipment, diapering procedures, posted information, staffing, supervision, interactions with children, caregiver/resident records, caregiver training, medication, emergency supplies and drill records, pet records, five records for children, attendance record, menu, lunch, indoor and outdoor areas. The children enjoyed a large selection of toys and learning materials for use in different interest areas. There were opportunities for individual choices, exploration and creative expression as well as guided group discussions for learning experiences.

Violations:
Standard #: 22VAC40-111-700-B
Description: Based on a review of the medication and related records kept at the family day home, and an interview with the provider, the provider did not obtain the parent's written authorization for one medication. Evidence: There was a physician's authorization form for use of a prescription medication for child 1M only in the event that it is needed and only the physician signed the form. The section for the name of the family day home and the parent's signature were left blank. The provider indicated that the parent submitted a form weeks ahead of enrollment of child 1M that had the parent signature and it was returned with a request to add a physician's signature. The physician's form was the only one provided the first date of attendance in August.

Plan of Correction: The provider immediately contacted the parents and will obtain the parent signature and make sure the name of the family day home operation is added to the permission form for the medication. In the future, the provider will review the medication form accepted from parents and make sure all required information and signatures are obtained before the form is filed.

Standard #: 22VAC40-111-720-A
Description: Based on a review of medication for children, and an interview with the provider, there was a prescription medication that was not labeled as required. Evidence: The prescription medication, to be used only as needed, was in a plastic bag with the last name of child 1M written on the bag. The medication was not in the original container with the prescription label identifying the child's name..

Plan of Correction: The provider immediately contacted the parents and will be provided the original box with the prescription container for the medication to be kept for child 1M during today's pick-up time. The provider will make sure the prescription medication brought for children always has the prescription label that includes the child's name.
