Standard #: 22VAC40-111-100-A
Description: Based on a review of nine children's files, the provider did not obtain documentation that each child has received a physical examination before the child's first attendance or within one month after attendance. Evidence: The file for Child #1 (date of first attendance: 12/28/17) did not have documentation of a physical examination.

Plan of Correction: Will ask the parent to obtain the missing documentation.

Standard #: 22VAC40-111-30-A
Description: Based on observation and interview, the provider failed to comply with all relevant state laws. Evidence: at the initial inspection, the provider was caring for nine children. State law requires family day homes to obtain a state-issued license from the Virginia Department of Social Services when caring for more than 4 children.

Plan of Correction: Will self-reduce to four children until license to operate has been received.

Standard #: 22VAC40-111-60-A
Description: Based on a review of nine children's files, it was determined that not all files were complete. Evidence: Child #5 and Child #6 did not have documentation of viewing proof of the child's age and identity in their file.

Plan of Correction: Have obtained the missing information and it is in their files.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider failed to ensure that all children's hands were washed as required. Evidence: after a diaper change, the provider put the infant on the floor to play without washing the child's hands.

Plan of Correction: Will ensure all hand washing procedures are followed.

Standard #: 22VAC40-111-690-E
Description: Based on observation, the provider failed to dispose of soiled diapers and wipes in such a way that the caregiver's hand nor diaper touches the exterior surface of the storage system during disposal. Evidence: after a diaper change, the provider first placed the soiled diaper on a shelf above the changing table. After placing the infant on the floor, the provider moved the soiled diaper to a foot operated leakproof storage system however she did not use her foot to open it. The provider's hand and the diaper touched the top of the storage system during disposal.

Plan of Correction: Moved the foot operated trash can next to the changing table.
