Comments:
A Subsidy Health and Safety inspection was conducted on 08/29/2019 to review supplemental health and safety requirements. The subsidy requirements were reviewed this date; allergy action plans, nutrition and food services, recordkeeping, hazardous substances and emergency plans. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on record review, the vendor did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: The provider's attendance does not include arrival and departure times.

Plan of Correction: Starting on Tuesday, I will use the attendance sheet that includes arrival and departure times.
