Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and follow up to previous inspection conducted on 03/19/2019. All previous violations were corrected.

Comments:
The following inspection took place between the hours of 9:30 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Chantilly, Virginia. At the start of this unannounced monitoring inspection there were a total of 11 children (28 points) with 2 caregivers and 1 volunteer. A total of 11 children?s files, 3 staff files, and 3 household member?s files were reviewed. The children were engaged in the following: toy play, games, art, story time, infant sleeping, bathroom breaks, hand washing, infant feeding, lunch, and other organized activities. Standard #: 22VAC40-111-180-A
Description: Based on review, 1 of 3 staff files indicated that the Provider did not obtain for each caregiver a current Report of Tuberculosis (TB) documentation (in accordance with the requirements in 22 VAC 40-111-170), every two years or more frequently as recommended by a physician or the local health department.

Plan of Correction: Current documentation shall be obtained and placed on file.
