Comments:
An unannounced monitoring inspection was conducted from 11:10am-12:20pm. There were 6 children (18 points) in the direct care of two caregivers. A sample size of 3 children's records, 2 caregiver records, emergency materials, evacuation drills, the day care space, and the playground were reviewed. There were no medications on site. The children were observed playing with toys, singing songs, and being served lunch. If you have any questions, please contact me at shahana.green@dss.virginia.gov .

Standard #: 22VAC40-111-200-A
Description: Based on a review of caregiver records, the Provider did not orient the assistant provider by the end of their first week of assuming job responsibilities. Evidence: 1. Caregiver #2 (doh: 5/16/2019) did not have documentation of an orientation on file.

Plan of Correction: I will give the orientation to the assistant provider and place in her file.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances were accessible to children. Evidence: 1. In the bathroom, the cabinet under the sink did not have a lock on it and contained cleaning supplies. 2. There were 4 sunscreens that stated "keep out of reach of children" on the back and were stored in the children's bins that were accessible to the children.

Description: Based on observation, the findings of the most recent inspection of the facility were not posted. Evidence: 1. The most recent inspection findings that were posted were from 11/30/2018. The most recent inspection of the facility was conducted on 5/13/2019.
