Comments:
An unannounced, non-mandated inspection was conducted from 11:20am-12:45pm. There were 6 children, plus the Provider's son, for a total of 7 children (20 points) with 3 caregivers. The Provider has hired a new Assistant and the record for the Assistant was reviewed, as well as the day care space. The children were observed dancing to music, being fed bottles, and laying down to take a nap. If you have any questions, please contact me at shahana.green@dss.virginia.gov .

Violations:
Standard #: 22VAC40-111-960-C
Description: Based on observation, not all infants who were not being held for feeding were place in a high chair, carrier seat, or feeding table. Evidence: 1. Child #1 was observed being placed in her crib and given a bottle to drink while in the crib by one of the caregivers.

Plan of Correction: This was corrected during the inspection and the infant was taken out of the crib for feeding. I will have caregivers attend infant training.

Standard #: 22VAC40-111-120-B
Description: Based on a review of staff records, one staff record was incomplete. Evidence: 1. Caregiver #1 was missing documentation of 1 out of 2 required references.

Plan of Correction: I will get a second reference for the assistant.
