Technical Assistance:
Reminder that every caregiver needs 16 hours of training annually.

Comments:
An unannounced renewal inspection was conducted today between 10:00 am and 1:00 pm, there were 9 children (23 points) with 2 caregivers. The children were observed playing with floor toys, coloring, handwashing, eating morning snack, and eating lunch. Lunch consisted of Chicken Nuggets, green beans, pineapple and milk was served. The physical plant, children's records, caregiver's records, evacuation drill logs, attendance logs, menu's, emergency plans, cribs and emergency supplies were inspected. Standard #: 22VAC40-111-780-B
Description: Based on a review of children's records, the provider failed to obtain annual acknowledgement of review of emergency contact information.

Evidence:
Child #2' s record did not contain acknowledgement of annual review of emergency contact.

Plan of Correction: I will get parent to read and sign today.

Standard #: 22VAC40-111-830-A
Description: Based on review of evacuation drill log, the provider failed to ensure that drills are practiced monthly.

Evidence:
There is no documentation for drill practice for 7/19 and 8/19.

Plan of Correction: Will conduct drills monthly.

Standard #: 22VAC40-111-100-A
Description: Based on review of children's record, the provider failed to ensure that each child has a physical within 30 days of their first day of attendance.

Evidence:
Child # 2, #4 and #5's files do not contain documentation of a physical 30 days after the first day of attendance.

Plan of Correction: I will get them from parents and put in files.

Standard #: 22VAC40-111-80-A
Description: Based on review of children's records, the provider failed to ensure that within seven business days of the child's first day of attendance to obtain verification of identity and age,

Evidence:
Child #4's record did not have any documentation of proof of identity.

Plan of Correction: I will get verification from parents and place in file.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to ensure that areas and furnishings of the family day home inside and outside shall be maintained in a clean, safe and operable condtion.

Evidence:
1) The wall behind the table where the children eat has had the paint scraped off of it.
2) The first white crib has has the paint chipping off.

Plan of Correction: I will do wall repair.
I will remove crib from family day home
