Comments:
An unannounced monitoring inspection was conducted today from 9:30am-11:30am. There were 9children (28 points) supervised by 3 direct caregivers. The home, 2 staff records, 9 children?s records, 1 household member?s records, evacuation drills, emergency, programming and policies were inspected. There were no medications on site today. Children were observed engaging freely and playing outside. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-111-780-B
Description: Based on review of 9 children's records, 4 do not have documentation of parental annual update to emergency contact information.
Evidence:
Children #1, 3, 4 & 5 do not have documentation of annual parental updates. Last updates were:2/1/18, 10/31/17, 5/2/18, & 7/18/18 consecutively.

Plan of Correction: I will have the parents update the emergency contacts this week.

Standard #: 22VAC40-111-100-B
Description: Based on review of 9 children's record, 1 does not have documentation of a physical examination within 30 days of attendance.
Evidence:
Child #2 start date, 7/2/18, does not have documentation of a physical examination.

Plan of Correction: Not available online. Contact Inspector for more information.

Standard #: 22VAC40-111-90-C
Description: Based on review of 9 children's records, 2 do not have documentation of an updated immunization record every 6 months for children under the age of 2.
Evidence:
Children #1 & 6 do not have documentation of an updated immunization record. Last updates were 2/1/18 & 8/24/18 consecutively.

Plan of Correction: I will get it from the parents this week.

Standard #: 22VAC40-111-180-A
Description: Based on review of 3 caregivers records, 1 does not have a current tuberculosis (TB) report on file.
Evidence:
Staff #1 has an expired TB report on file, expiration date is 8/23/19.

Plan of Correction: Staff has an appointment to get an xray for her TB report.

Standard #: 22VAC40-111-430-B
Description: Based on observation, a pressure mounted gate is being used as a protective barrier at the stair opening.
Evidence:
At the bottom on the stairs, a pressure mounted gate is used at the bottom of the stairs located between the playroom and bathroom.

Plan of Correction: We usually keep the door closed so the children can't go by the stairs.

Standard #: 22VAC40-111-570-A
Description: Based on observation, caregiver exceeded 16 points.
Evidence:
Upon arrival the inspector observed 7 children (18 points) outside with 1 caregiver.

Plan of Correction: There was a second caregiver with 1 child inside getting them ready to go outside. I believe they were alone with 1 caregiver for less than 2 minutes.
