Comments:
An unannounced monitoring inspection was conducted on 08/28/2019 from 2:30 P.M.-4:45 P.M. At the time of arrival two children were in the care of one provider, an additional eight children arrived afterschool and another caregiver arrived. Total of ten children present (eleven points) were in the care of two caregivers. Five children's records, two caregivers records, evacuation drills, first aid kit, emergency supplies, emergency plan, required posted information, and playground were reviewed. Children were observed during quiet activities, television time watching cartoons, arrival from afterschool, handwashing,snack time, free choice activities that included dramatic play and building, and departure. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-111-800-B
Description: Based on review of the emergency plan, the provider failed to review the emergency plan annually.

Evidence:
1. The last recorded date of annual review was dated 06/05/2018.
2. The provider verified that the emergency plan had not been updated since 06/05/2018.

Plan of Correction: The provider will complete an update on emergency plan immediately.

Standard #: 22VAC40-111-100-A
Description: Based on a review of children records, the provider failed to obtain a physical examination prior to a child's first day of attendance or within 30 days after the first date of attendance.

Evidence:
1. Child #2's file did not have a physical examination on file. Date of enrollment was 03/11/2019.
2. The provider verified that there was not a physical examination on file,and the date of enrollment for Child #2.

Plan of Correction: The provider will contact the parent and ask that the physical examination be provided.

Standard #: 22VAC40-111-60-B
Description: Based on review of children's records, the provider failed to obtain complete enrollment information for two out of five files reviewed.

Evidence:
1. Child #1 and Child #2's file did not have a written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately completed. Date of enrollment for Child #1 and Child #2 was 03/11/2019.
2. The provider verified that the written authorization for emergency medical care had not been completed for Child #1 and Child #2.

Plan of Correction: The provider will contact the parent and obtain the written authorization for medical care.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children records, the provider failed to obtain documentation that a child had been adequately immunized before the first day of attendance.

Evidence:
1. Child #2 did not have any documentation of immunizations on file. Date of enrollment for Child #2 was 03/11/2019.
2. The provider verified that there was no documentation of immunizations in the child's record and date of enrollment of Child #2.

Plan of Correction: The provider will speak to the parent and request the immunizations be provided.
