Comments:

An unannounced Renewal inspection was conducted from 11:00am-1:45pm. There were 6 children (20 points) in the direct care and supervision of 3 caregivers. A sample size of 5 children's records, 3 caregiver records, 2 household member records, the first aid kit, evacuation drills, emergency materials, the playground, and the daycare space were reviewed. Description:	Based on a review of records, 1 out of 2 Assistant's records were incomplete. Evidence: 1. Caregiver #2 (hire date: 6/1/2018) was missing documentation of 2 of 2 required references.
Plan of Correction:	I will ask her for references and call them as soon as possible.
Standard #:	22VAC40-111-60-A
Description:	Based on a review of children's records, 1 out of 4 records was incomplete. Evidence: 1. Child #4 was missing an annual update within the last year. The last annual update was conducted on 9/6/2018.
Plan of Correction:	I will ask the grandpa to sign today.
Standard #:	22VAC40-111-200-A
Description:	Based on a review of staff records, the substitute provider was not oriented. Evidence: 1. Staff B (hire date: 6/1/2018) does not have documentation of orientation on file.
Plan of Correction:	I am going to go over the orientation with her today.
Standard #:	22VAC40-111-530-A
Description:	Based on observation, children other than infants did not have linens consisting of a top and bottom cover when napping. Evidence: 1. Child #4 was missing a top cover during the designated rest period. 2. Child #5 was missing a top and bottom cover during the designated rest period.
