Comments:
A Subsidy Health and Safety inspection was conducted from 11:00am-1:45pm to review supplemental health and safety requirements. There were 6 children (20 points) in the direct care and supervision of 3 caregivers. A sample size of 5 children's records, 3 caregiver records, 2 household member records, the first aid kit, evacuation drills, emergency materials, the playground, and the daycare space were reviewed. The children were observed playing, looking at books, being fed bottles, and taking a nap.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Violations:
Standard #: 22VAC4-665-340-I
Description: Based on observation, pillows were used by children under two years of age while resting or sleeping.
Evidence:
1. Child #5 (dob: 10/6/2017) was observed laying down for the designated rest period with a pillow.

Plan of Correction: We removed the pillow during the inspection and will not give it to the child.

Standard #: 22VAC40-665-230-B
Description: Based on a review of caregiver records, 1 out of 3 caregivers did not complete the Virginia Preservice Training within the first 90 days of employment.
Evidence:
1. Caregiver #3 does not have documentation of completing the Virginia Preservice Training on file.

Plan of Correction: I will ask the employee to take the training within two weeks.

Standard #: 22VAC40-665-410-A-3
Description: Based on a review of evacuation records, lock down procedures were not practiced annually.
Evidence: There was no record of a lock down procedure being practiced during the past year.

Plan of Correction: We will practice it and document it.
