Comments:

An unannounced monitoring inspection was conducted between approximately 11:00am and 1:20pm. 9 children (30pts) were present with 3 caregivers. Ratios were in compliance. The Provider was not onsite but arrived at 12pm, 1 hour after the inspection began. Upon arrival, the licensing inspector observed the children divided into 2 groups. One group was outside and one was inside. Some had just woken up from a nap. The children outside were observed coming inside to wash hands before eating outside (lunches are sent in from home and labeled). The children inside were engaged in tummy time, finishing eating and interacting with caregivers. Interactions were positive. The diapering procedure was observed and in compliance. The physical space, materials and activities, 4 children's records, caregiver records, evacuation drills, emergency supplies and attendance records were reviewed. Required posted documents were in compliance. 3 cribs were inspected and in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #:	22VAC40-111-510-A
Description:	Based on observation and interviews with the caregivers, an individual crib was not provided for each child. Evidence: 3 cribs were available and 5 infants were present today.
Plan of Correction:	I will make sure each infant has an individual crib from now on. (The Provider ordered 2 cribs online while the Licensing Inspector was present.)
Standard #:	22VAC40-111-520-A
Description:	Based on observation and interview with caregivers, cribs were not provided for children from birth through 12 months of age. Evidence: 5 infants were present but only 3 cribs were available for use. The 2 infants not in cribs were awake, being held and in bouncy seats. The Provider stated their nap time is later.
Plan of Correction:	(The Provider ordered 2 cribs online while the Licensing Inspector was present.) I will make sure to have individual cribs for each infant from now on.
Standard #:	22VAC40-191-50-A-3-a
Description:	Based on record review and interview with the Provider, Caregiver #1 did not have fingerprints (a Criminal History Record) completed and available for review. Evidence: Caregiver #1 started on 8/18/2019. Other required documents were completed but the fingerprints were scheduled for 9/13/2019. The Provider stated she waited for the results of the Central Registry Search to be returned because she thought that was the Criminal Record Check that needed to be completed prior to starting.
Plan of Correction:	I will reschedule the appointment and have her go today. She will not return until the results have been received.
