Comments:
A subsidy health and safety inspection was completed on August 21, 2019 from approximately 12:45 p.m. to 2:30 p.m. to review supplemental health and safety requirements. Seven children, infants, were in care for a total of twenty-six points, with two caregivers. Caregivers and children were observed in the indoor space engaged in activities to include daily activities for infants. Interviews and interactions were conducted and no medications are given. Five children?s records and two caregiver records were reviewed. The Provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-230-B
Description: Based on a review of records, the licensee failed to ensure that within 90 days of employment all caregivers shall complete Virginia Preservice Training for Child Care Staff for one of two caregivers.
Evidence: 1. Caregiver #2's record, hired on September 4, 2018, lacked documentation of completion of Virginia Preservice Training for Child Care Staff. 2. Caregiver #1 acknowledged that Caregiver #2 has not competed this training within the required time frame of hire.

Plan of Correction: The provider will ensure that Caregiver #2 completes this training by the end of the month.

Standard #: 22VAC40-665-230-E
Description: Based on a review of records, the licensee failed to ensure that within 90 days of employment all caregivers shall obtain current certification in CPR and first aid for one of two caregivers.
Evidence: 1. Caregiver #2's record, hired on September 4, 2018, contained documentation in which CPR and first aid was completed on February 27, 2019 and therefore not within the required time frame. 2. Caregiver #1 acknowledged that Caregiver #2 did not complete these certifications within the required time frame of hire.

Plan of Correction: This cannot be corrected.

Standard #: 22VAC40-665-410-A-3
Description: Based on a review of records, the licensee failed to practice lockdown procedures at least annually.
Evidence: 1. A review of the practice drill log lacked documentation of a practice lockdown drill for the past year. 2. Caregiver #1 acknowledged this drill was not practiced at least annually as required.

Plan of Correction: The provider will get a planner that includes a calendar and will begin tracking this date annually to ensure the drill is practiced.
