Comments:
A renewal inspection was conducted September 9 2019 between 10:00am and 12:00pm. There were 5 children ( 18 points) in the direct care of the provider and an assistant. Children were observed napping, playing with age-appropriate toys, reading, singing, and bouncing in a bounce seat. The day care space is clean and organized. Standard #: 22VAC40-111-70-A
Description: Based on record review, 1 of 5 children's records reviewed did not contain documentation that the parents received, in writing, all of the required documents prior to the child's first date of attendance.

Evidence: Child #4 record did not contain the information for parents form, medication policy form, liability insurance form and the provisions for emergency response form.

Plan of Correction: When the child is picked up I will have the forms ready for the parent to sign.
