Technical Assistance:
Licensing inspector discussed renewal application, and requirements for subsidy providers.

Comments:
An unannounced renewal inspection was conducted today from approximately 10am-11:40am. There were 3 children (10 points) directly supervised by 1 caregiver. A sample of 2 children?s records, 1 caregiver's record and 2 household member's records were reviewed. The children were observed napping and engaged with the provider. Zero new caregivers have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Violations:
Standard #: 22VAC40-111-60-A
Description: Based on interview, the provider did not maintain a complete record for each child enrolled.

Evidence:
1. Caregiver A did not have a complete record for each child in care.
2. Caregiver A stated Child A was a family member and only had her shot records.
3. Child A was missing the following required information: Name and address of both parents, employer and employer phone number of both parents, 2 of 2 required emergency contacts, health insurance company and policy number, parent agreement, liability insurance statement,

Plan of Correction: Child A is my niece. I will ask the mother to complete the forms.

Standard #: 22VAC40-111-320-A
Description: Based on observation, small electrical appliances were not in use and plugged in during childcare hours.

Evidence:
A toaster and a blender were both plugged in on the kitchen counter.

Plan of Correction: Corrected during inspection.
