Comments:
An unannounced monitoring inspection was completed on 9/12/2019. There were five children in care with the licensee. The point total for the five children was 12. Five children's records were reviewed, the provider's record was reviewed, and the household member's record was reviewed. No medications at the family day home.

The inspector arrived at the provider's home at 9:10 am and departed at 11:30 pm.

The children were observed during painting, free time, during a music and movement activity, story time..

Discussion with licensee about: update for the CPS background check for 2020.

Violations:
Standard #: 22VAC40-111-60-A
Description: Based on review of five children's records, the center failed to ensure that all records were complete with all information required by the standard.

Evidence:
1. Children # 2 and 3 were missing the insurance policy number.
2. Child # 5 was missing one of the two required emergency contact and start date.

Plan of Correction: The licensee will have the parent to complete the missing information. All records will be reviewed.
