Discussed infant/toddler naptime and crib items.

Comments:
An unannounced renewal inspection was conducted today from 11:30am-2:45pm. There were 11 children (36 points) supervised by 2 direct care staff. The home, 2 staff records, 6 children?s records, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-940-A
Description: Based on observation, the provider did not ensure that refrigerator compartments were maintained at or below 40 degrees Fahrenheit.

Evidence: The refrigerator temperature was reading 48 degrees Fahrenheit.

Plan of Correction: We will replace the refrigerator with a new one.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 2 of 6 children's records did not contain documentation of all required information.

Evidence: Child #10 and Child #11's records did not contain documentation of immunizations and parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70

Plan of Correction: I will get the missing forms from parents.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider did not ensure that potentially poisonous substances were stored away in areas inaccessible to children.

Evidence:
1. In the large play area, 2 spray bottles containing chemical cleaning solutions were found in a metal basket by the cubby shelves.
2. In the infant room, 1 spray bottle containing chemical cleaning solution was found hanging under a shelf next to the diaper changing station.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-570-A
Description: Based on observation and record review, the provider did not ensure that the family day home does not exceed 16 points per caregiver.

Evidence: The family day home had 36 points with two caregivers.

Plan of Correction: One assistant got sick today and left in an emergency.

Standard #: 22VAC40-111-680-A
Description: Based on observation, 1 of 2 caregivers did not ensure that their hands were washed with liquid soap after diapering.

Evidence: Caregiver B did not wash their hands after changing two children's diapers.

Plan of Correction: The caregiver was nervous because the licensing inspector was present. I have talked with the assistants about not getting nervous and following all regular procedures when the inspector is present.

Description: Based on observation, 1 of 2 caregivers did not ensure that children's hands were washed after diapering.

Evidence: Caregiver B did not wash Child #5 and Child #7's hands after changing their diapers.

Plan of Correction: The caregiver was nervous because the licensing inspector was present. I have talked with the assistants about not getting nervous and following all regular procedures when the inspector is present.

Standard #: 22VAC40-111-690-D
Description: Based on observation, 1 of 2 caregivers did not ensure that the diapering surface was cleaned and sanitized after each use.

Evidence: Caregiver B changed two children's diapers and did not clean and sanitize the diapering surface after each diaper change.

Plan of Correction: The caregiver was nervous because the licensing inspector was present. I have talked with the assistants about not getting nervous and following all regular procedures when the inspector is present.
