Comments:
An unannounced monitoring inspection was conducted today from 10:15am-12:15pm. There were 4 children (10 points) supervised by 1 direct care staff. The home, 1 staff records, 2 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing with Legos, animals, and engaging freely. Diapering and handwashing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-180-A
Description: Based on record review, 2 of 3 caregivers and household member records did not contain documentation of current reports of TB screening forms every two years.

Evidence:
1. Caregiver A's TB was dated 9/9/2017 (expired: 9/9/2019).
2. Household member B's TB was dated 9/5/2017 (expired: 9/5/2019).

Plan of Correction: We will get updated TB's this week.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider did not ensure that potentially poisonous substances were stored in areas inaccessible to children.

Evidence: A cabinet in the second childcare room had an unlocked cabinet that contained 5 containers of Lysol spray and Febreeze on the bottom two shelves.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-320-G
Description: The provider did not maintain annual inspections for wood burning fireplaces.

Evidence: The most recent fireplace inspection was conducted on 3/23/2019 and noted the fireplace as a possible fire hazard.

Plan of Correction: I will make an appointment today.

Standard #: 22VAC40-111-680-A
Description: Based on observation, the provider did not ensure that caregivers washed their hands with liquid soap and warm running water after diapering a child or assisting a child with toileting.

Evidence: After changing one child's diaper, Caregiver A did not wash their own hands with liquid soap and warm running water before changing the second child's diaper.

Plan of Correction: I will be sure to wash hands after every diaper change.

Description: Based on observation, the caregiver did not ensure that the diapering surface was cleaned and sanitized after each use.

Evidence: After changing one child's diaper, Caregiver A did not clean and sanitize the diapering surface before changing the second child's diaper.

Plan of Correction: I will be sure to clean the diapering pad after each diaper change.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the provider did not ensure the findings of the most recent inspection of the facility were posted.

Evidence: The most recent inspection posted was dated 10/9/2018. The most recent inspection was conducted on 3/4/2019.

Plan of Correction: I was waiting for the inspection to come in the post office mail. In the future, I will follow up with my inspector if I don't receive it.
