Technical Assistance:

Licensing inspector discussed with provider training opportunities, Medication Administration Training, hand-washing, and stationary equipment.

Comments:

An unannounced renewal inspection was conducted today between the hours of 10:25 am through 1:45 pm with the provider on site. There were 10 children (29 points) with 2 caregivers. A sample of 4 children's files, 2 caregiver files, and 4 household member's files were reviewed. The menu, attendance records, evacuation drills, the emergency plan, and first aid kit were also reviewed. The children were engaged in the following: toy play, movie time, diapering, hand washing, lunch, naptime and other organized activities. Interactions between the children and staff were positive. Description:	Based on observation and caregiver interview, the emergency plan was not reviewed annually. Evidence: 1. The last documented review of the emergency plan was dated 4/13/2015. 2. Caregiver reported that the plan had not been reviewed annually.

Standard #:	22VAC40-111-170-A
Description:	Based on review of household members records, the provider did not obtain Report of Tuberculosis Screening from all adult household members. Evidence: 1. 1 of 4 household member records did not contain an initial tuberculosis screening. 2. The record for household member 1 did not contain an initial tuberculosis screening.

Description:	Based on review of household members records, the provider did not obtain current Report of Tuberculosis Screening from all adult household members. Evidence: 1.1 of 4 household member records did not contain a current tuberculosis screening. 2. The tuberculosis screening of household member 2 was last completed January 18th 2014.
