An unannounced monitoring/renewal inspection was conducted today from 11:45am-1:15pm. There were 5 children (14 points) directly supervised by the provider. The home, 1 staff records, 5 children?s records, 1 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely, painting, dancing, playing with instruments and the infant in care was observed to be held while bottle feeding. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Evidence:
The record for Child 2 did not contain documentation of a physical examination. The child's first day of attendance is listed as 7/15/19.Plan of Correction: I will ask the parents to provide documentation of a physical.
