Evidence: The last documented review of the provider's emergency preparedness plan is dated 3/22/2018. The provider stated that she had not reviewed the emergency plan.Plan of Correction: I will read it and resign.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's record did not contain all required information.
Evidence: Child #1's file did not contain complete work contact information for one parent. Child #3's file did not contain the family's medical insurance policy number.Plan of Correction: I will request it from the parents today to get the information tomorrow.
Standard #: 22VAC40-111-280 Description: Based on observation, potentially poisonous substances were not inaccessible to children.
Evidence: There were two air fresheners located on the top of the toilet tank in the childrens bathroom.Plan of Correction: One of them is empty, I will throw that one out. I will take the other one out.
Standard #: 22VAC40-111-430-B Description: Based on observation, pressure mounted gates were used as a protective barrier at stair openings.
Evidence: A pressure mounted gate is used at the bottom of the stairs that leads to the childrens play area.Plan of Correction: I will do it this weekend.

Evidence: Three out of five children napping on rest mats did not have a bottom linen between the mat and the child.Plan of Correction: I already have a crib sheet that I will put on there tomorrow.
