Violations:
Standard #: 22VAC40-111-800-B
Description: Based on a review of the emergency preparedness plan, the licensee failed to review the plan at least annually and update the plan as needed.
Evidence: 1. The last recorded update of the emergency preparedness plan was observed and dated December 2015. 2. Caregiver #1 stated that the plan has been updated since then but acknowledged it has not been annually as required.

Plan of Correction: The provider will update the plan now and then update it annually as required.
