An unannounced monitoring inspection was conducted today between the hours of 9:15am through 10:45am. There were a total of 5 children present ages 15 months to 4 years (13 points) with the Provider and one assistant. A total of 5 children's files, 2 staff files, and 2 household member's files were reviewed. The children were engaged in the following: reading books and participating in organized center play. Positive interactions between caregivers and children were observed during this inspection. A complete inspection of the programming space, children and staff records, evacuation drills, menu, attendance, and emergency procedures were completed today. The provider advised there was no medication on site today. Both staff present are current in CPR/First Aid certification. There was an abundant supply of age appropriate materials and toys. Standard #: 22VAC40-111-60-B Description: Based on review of 5 children's records, 5 children's records did not contain documentation of an annual review of emergency contact information completed by the children's parents.
Evidence:
1. The last review of emergency contact information for Child #1 was dated 8/15/18, Child #2 was dated 12/18/17, Child #3 was dated 8/24/17, Child #4 was dated 8/1/18, and Child #5 was dated 12/23/15.Plan of Correction: I will have all parents review and sign the update today.
Standard #: 22VAC40-111-90-C Description: Based on review of 5 children's records, 2 children's records did not contain documentation of updated immunizations once every six months for children under the age of 2.
Evidence:
1. Child #2 is under the age of 2 and the most recent immunizations on file were dated 12/6/17.
2. Child #4 is under the age of 2 and the most recent immunizations on file were dated 8/7/18.Plan of Correction: I will tell parents today to bring updated immunizations.
Standard #: 22VAC40-111-180-A Description: Based on review of 2 adult household member records, 2 records did not contain documentation of a tuberculosis screening or test being completed within the last 2 years.
Evidence:
1. Adult Household Member #1's last tuberculosis test was completed 4/6/17.
2. Adult Household Member #2's last tuberculosis test was completed 5/11/17.Plan of Correction: I'm going to tell them to go and do the TB test.
