Technical Assistance:
Licensing Inspector discussed with the provider the following: record keeping, diapering area, and nap time.

Comments:
An unannounced renewal inspection was conducted today between the hours of 11 am through 3:30 pm with the provider on site. At the beginning of the inspection, there were 9 children for 26 points with the provider and two assistants present. A sample of 5 children's files, 3 caregiver files, and 2 household member's files were reviewed. The children were engaged in the following: playing with toys,playing outdoors, eating lunch, and naptime. Diapering and hand-washing were also observed. Interactions between the children and caregivers were positive. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Violations:
Standard #: 22VAC40-111-900-B
Description: Based on observation, a child was drinking while lying down.
Evidence:
An infant was observed being fed a bottle while laying down in an infant seat.

Plan of Correction: Corrected during inspection. I will tell my staff not to feed infants who are lying down.

Standard #: 22VAC40-111-60-B
Description: Based on review of children's records, each record did not contain the required information.
Evidence:
1. The record for Child #1, 2, 3, 4, and 5 did not contain the signature of the person verifying proof of identification.
2. The record for Child # 3 (start date 6/16/17) and #5 (start date 5/21/18) did not contain documentation of the review of the child's emergency contact information annually.
3. The record for Child #1 (start date 12/04/17) did not contain results of a health exam.

Plan of Correction: I will update the files.
I will ask the parents to update the emergency information.
Provider stated she gave the health exam documentation back to Child #1's mother due to an upcoming appointment with a doctor. I will ask mom for the documentation.

Standard #: 22VAC40-111-240-A
Description: Based on observation, some areas and furnishings, inside and outside, were not maintained in a safe condition.
Evidence:
1. A piece of fabric was being utilized to keep a cabinet closed, creating hanging fabric at child height in the play area, accessible to children.
2. Cords from sound machines utilized during naptime were observed stretching across the floor creating a tripping hazard.
3. Peeling paint was observed on the exterior wall of the house at child height.
4. One board on the fence surrounding the play area contained sharp edges;another had areas of splintering at child height.

Plan of Correction: I will use a child lock to secure the cabinet today.
I will use batteries for the sound machines today.
I will make sure repairs are completed.

Standard #: 22VAC40-111-450-A
Description: Based on observation, doors with clear glass panels that reach within 18 inches of the floor were not clearly marked with decorative objects.
Evidence:
The sliding glass door exiting to the outdoor play area was not clearly marked with decorative objects.

Plan of Correction: I will put paint on the door.

Standard #: 22VAC40-111-510-A
Description: Based on observation today, each child was not provided with a cot or rest mat for resting or napping.
Evidence: 4 of 9 children were observed sleeping today in bounce seats.

Plan of Correction: They usually are sleeping on mats in the floor. I will make sure the children are not sleeping in the bounce seats.

Standard #: 22VAC40-111-510-C
Description: Based on observation, caregiver did not ensure rest mats were at least 12 inches from each other.
Evidence: During naptime, two children were observed sleeping in individual rest mats that were approximately 4 inches apart.

Plan of Correction: I will make sure there is space between the mats.

Standard #: 22VAC40-111-530-B
Description: Based on observation, one crib in use did not have a tight-fitting bottom cover.
Evidence:
1.The crib closest to the wall of the left side of the infant room contained a crib sheet that was not tight-fitting and allowed material to gather on the surface of the mattress.
2. The caregiver stated the infant was sleeping in the crib and had just woken up.

Plan of Correction: I will tell the parents to bring a new sheet tomorrow that fits tighter.
