Technical Assistance:
Provider was informed that their MAT certification expired in October. Tight fitting sheets for cribs, disposing of topical ointments for children no longer enrolled, ensuring proper documentation of reviewing proof of identity to include who viewed the document and the date in which it was viewed.

Comments:
An unannounced monitoring/renewal inspection was conducted today from 9:30am-11:00am. There were 7 children (17 points) directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating lunch. There was a sufficient supply of books, toys, and materials for the children. A trampoline was observed in the back yard. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on Review of Child Records it was determined that all required information was not maintained
Evidence: Child 1's record did not contain documentation of provisions for emergency plan, liability insurance
Child 2's record did not have a signed acknowledgement of a medication policy by the parent.

Plan of Correction: Provider will get the information from parent.

Standard #: 22VAC40-111-250-B
Description: Based on observation, children under the age three years old were not protected from toys less than 1-1/4 inches in diameter and less than two inches in length.

Evidence: During inspection an 8 month old child was playing with action figures from a container which contained small toys less than 1-1/4 inches in diameter and less than two inches in length with assistant present.

Plan of Correction: Items were removed during inspection.

Standard #: 22VAC40-111-560-A
Description: Based on observation, Provider did not make sure that all caregivers were aware of the location of children to be in sight and sound at all times.

Evidence: Provider informed inspector(s) that she was going upstairs to check on lunch but did not inform assistant that 1 child was in the bathroom.

Plan of Correction: Usually assistant helps with lunch preparation but because 1 child was bothered by the inspectors the assistant was in another room. I will continue to communicate with assistant and take some of the children with me when necessary.
