Technical Assistance:
The following technical assistance was provided during the inspection.: How to determine points and be aware of how many children and total points are in care at any given time.

Comments:
An unannounced monitoring inspection was conducted between 8:00am and 10:00am. Upon arrival there were 5 children in care, totaling 10 points and 1 caregiver. All five children's records were reviewed. Children were observed playing outside on play equipment and pretending to "go to work."

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on a record review, the licensee did not ensure all required information was kept for each child.
Evidence:
1. Two year-old, Child #1 (start date 05/22/18) did not have documentation of having a physical examination.
2. Two year-old child #2 did not have 2 complete emergency contact information, to include the addresses.
3. Three year-old child #3 did not have documentation of his first date of enrollment.

Plan of Correction: The following corrective action was provided by the licensee during the exit interview:
The records will be corrected and all required information will be documented in each child's file.

Standard #: 22VAC40-111-80-B
Description: Based on a record review the licensee did not ensure the identity and age of each child was verified.
Evidence
Two year-old child #4 (start date 10/15/18) did not have documentation in the record that the licensee verified the identity and age of the child.

Plan of Correction: The following corrective action was provided by the licensee during the exit interview:
The licensee will contact the child's parent today and request to see proof of identity and will document the viewing in the child's record.
