Technical Assistance:

Inspector and provider discussed medication authorization forms.

Comments:

An unannounced renewal inspection was conducted today from 11:00am-12:45pm. There were 6 children (22 points) directly supervised by 2 staff. The home, 2 staff records, 2 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed reading books, playing freely, playing in the sandbox, strolling in the stroller and eating lunch. There was a supply of books, toys, and materials for the children. Standard #:	22VAC40-111-170-A
Description:	Based on record review, each adult household member did not have documentation of the absence of tuberculosis in a communicable form. Evidence: Household member 1 does not have documentation of the absence of tuberculosis in a communicable form. (Household member 1 move in date: 9/6/2019)
Plan of Correction:	The household member was told about the tuberculosis screening requirement. We will ensure it gets completed as soon as possible.
Standard #:	22VAC40-191-60-C-2
Description:	Based on record review, all caregivers did not have required background checks within 30 days of employment. Evidence: Staff 1 does not have a completed central registry background check. (Staff 1, date of hire: 7/8/2019)
Plan of Correction:	The background check was sent on 7/8/2019. I will follow up with an email to the background check office and will get an update.
Standard #:	63.2-1721.1-B-2
Description:	Based on record review, all household members did not have a completed fingerprint based criminal history record check. Evidence: Household member 1 does not have background check results for a fingerprint based criminal history record check. (Household member 1, move in date: 9/6/2019)
Plan of Correction:	I have been in touch with my household member. I will ensure the background check gets completed.
