Comments:
An unannounced monitoring inspection was conducted today between the hours of 11:45am through 12:25pm with the provider on site. There were 5 children in care (14 points) with the provider. A sample of 3 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play and eating lunch. Interactions between the children and caregiver were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Description: Based on interview, the provider did not obtain updated tuberculosis (TB) tests or screenings for each household member every two years.
Evidence:
1. The provider stated she did not have the updated TB test for household member #1.

Plan of Correction: I will have the household member get an appointment.
