The provider shall send documentation of all completed training hours for herself and assistant caregiver A no later than 10/4/19.
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 7 children, totaling 14 points. Children's records, caregiver records, household member records, health records, training documentation, background checks, administrative records, emergency preparedness items, the physical plant, children's nutrition, and child care programming were inspected today.
The children were observed engaging in creative play activities and interacting with both caregivers throughout the course of this inspection.
The children were to be served chicken/vegetable fried rice, apple slices, and milk for lunch today.

Standard #: 22VAC40-111-800-B Description: Based on a review of administrative documents, it was determined that the provider has not completed an annual review of the emergency preparedness plan.
The provider most recently reviewed the emergency plan on 9/18/18.
Plan of Correction: I will review the plan with my assistant during nap time today.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that four records were not complete.
1 - Child records A, B, and C do not include each parent's medical insurance information.
2 - Child record A does not include each parent's employment contact information.
3 - Child record D does not include documentation that the parent has reviewed the child's file within the past year. This parent most recently reviewed the file on 9/4/18.
Plan of Correction: Each parent will be asked for the missing information.
Standard #: 22VAC40-111-320-B Description: Based on observations, it was determined that two electrical outlets were not covered.
Two outlets next to the children's handwashing sink were not covered.
Plan of Correction: I will cover them today.
Standard #: 22VAC40-111-690-E Description: Based on observations, it was determined that the provider has not installed a liner inside the diaper trash can.
There is no plastic liner inside the diaper trash can.
Plan of Correction: I will put the plastic bag inside the trash can before we do another diaper change.
