Comments:
An unannounced monitoring inspection was conducted today between the hours of 11:25am through 12:45pm with the provider on site. There were 4 children (8 points) with the provider. A sample of 4 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: coming in from a walk at the park, toy play, looking at books, washing hands and lunch time. Interactions between the children and provider were positive. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-90-A
Description: Based on review of four children's records, the provider did not obtain documentation of immunizations for a child prior to the child's first day of attendance.
Evidence:
Child #1 began with the provider on 9/16/2019 and the provider did not have the child's immunization record.

Plan of Correction: I called the parent and requested the immunization record during the inspection.

Standard #: 22VAC40-111-330-A
Description: Based on interview, the provider did not have a landline telephone that is available, operable and accessible during the family day home hours of operation.
Evidence:
1. The provider stated she has a cell phone, but does not have a landline operable phone in the home at this time.

Plan of Correction: I will get a landline phone at my new location.
