Technical assistance was provided regarding cleaning and sanitizing equipment and toys, physical plant, record keeping and the license.
An unannounced focused monitoring inspection was conducted today from 10:00am-11:00am. There was 1 child (4 points) directly supervised by 1 staff. The home, 1 staff records, 3 children?s record and physical plant. Children were observed holding their own bottle and watching television. Diapering procedures were also observed. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-60-B Description: Based on record review, the provider failed to ensure all required items were in the child's record.
Evidence: Child 1 is missing one parent work phone number.
Plan of Correction: I will follow up with the parent.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not obtain documentation all children have been adequately immunized.
Evidence: Child 1 does not have documentation of immunizations. (Child 1, start date 9/17/2019)
Plan of Correction: The parents have been provided the forms. They will bring back to me after they go to the doctor.
Standard #: 22VAC40-111-90-C Description: Based on record review, the provider did not obtain additional immunizations every 6 months for children under the age of 2.
Evidence: Child 2 has immunizations which are dated 1/3/2019 and is under 2 years of age.
Plan of Correction: I will ask parents for updated immunization records.
Standard #: 22VAC40-111-240-A Description: Based on observation, areas outside of the home were not kept in a safe condition.
Evidence: On the outside back porch, two wood beams of the porch have rotted and worn away exposing the nails underneath causing a potential safety hazard to children.
Plan of Correction: The board have been ordered. We are waiting for the boards to be repaired.
