Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and training. The Provider and Assistant are both schedule to obtain MAT certification, on 09/23/2019 for the Assistant, and 10/04/2019 for the Provider. The Provider's certification expired 06/18/2019. Currently there are no medications on site. The Provider has also informed the parents of her certification status.

Discussion that the chimney will be reinspected this week. The last chimney inspection was conducted on 09/25/2018.

Comments:
The following inspection took place between the hours of 8:30 am through 11 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 2 children (6 points) with 2 caregivers. A total of 5 children?s files, and 3 staff files were reviewed. The children were engaged in the following: toy play, games, morning snack, tummy time, infant feeding, free play, learning, and other organized activities. Standard #: 22VAC40-111-780-B
Description: Based on review, 1 of 5 children files reviewed indicated that annually, the Provider did not review with the parent the emergency contact information (required in 22 VAC 40-111-60 B 2 to ensure that the information was correct), and obtain the parent's signed acknowledgment of this review.

Evidence -
1. On the date of inspection (09/18/2019), the date of the last review of file for Child #2 was listed as - 08/23/2018.
2. All other files reviewed for this standard, as of today, were in compliance.

Plan of Correction: This documentation shall be review, signed, and dated. If needed, information shall be updated.

Standard #: 22VAC40-111-90-C
Description: Based on review, 1 of 5 children files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (09/18/2019), the most current immunization documentation on file for Child #1 was dated - 10/26/2018.
2. Based on the information on file, Child #1 was under the age of two years on the date of inspection.

Plan of Correction: An updated copy of this immunization shall be obtained and placed on file.
