Technical assistance was offered regarding the following: Outdoor equipment, menu, children's immunization records and reviewing standards to maintain improved understanding and compliance.
- Comments:

An unannounced monitoring inspection was conducted from 11:00 am to 1:10 pm. There were 5 children (16 points) in care of three caregivers. The children were observed playing on the outside play area, getting diapers changed and eating lunch outdoors. The lunch menu included rice, chicken, mixed vegetables and yogurt. The children were also observed during nap time. There were no medications on site. The emergency drill records were current. 5 children's records, 3 caregiver records and 2 household member records were reviewed. Standard #: 22VAC40-111-500-B Description: Based on observation, the highest climbing platform on a stationary climbing equipment exceeded 4 feet in height.
Evidence:
1. At the beginning of the inspection, Child # 1 was observed on the top of the wood tree house equipment. Child # 1 was observed standing on bench on the platform.
2. The height of the platform where Child # 1 was measured at 7 feet and 10 inches.Plan of Correction: The equipment will no be longer be used by the children. We will remove the ladder to ensure that it is no longer being used.
Standard #: 22VAC40-111-680-A Description: Based on observation, caregivers did not wash hands after changing a diaper.
Evidence: Caregiver # 1 changed a diaper for Child # 1. Caregiver did not wash her hands with warm running water and soap after the diaper change. Caregiver # 1 was observed using hand sanitizer instead.Plan of Correction: We will retrain the caregiver and ensure proper handwashing is done.
Standard #: 22VAC40-111-680-B Description: Based on observation, caregivers did not ensure that children's hands were washed after diaer changing.
Evidence: Caregiver # 1 did not wash Child # 1's hands with warm running water and soap. Child # 1 was observed moving on with other activities right after the diaper change.Plan of Correction: We will make sure that children wash hands after toileting and diaper changing.
Standard #: 22VAC40-111-690-D Description: Based on observation, the diapering surface was cleaned and sanitized after every diaper change.
Evidence:
1. Caregiver # 1 was observed changing a diaper for Child # 1.
2. After the diaper was changed, Caregiver did not use soap and water to clean the diaper pad. She only used the sanitizing solution.Plan of Correction: We will ensure that the two steps of cleaning and sanitizing are followed.
