Comments:
A Subsidy Health and Safety inspection was conducted on 9/27/19 to review supplemental health and safety requirements.
The subsidy requirements were reviewed this date; allergy action plans, staff trainings, nutrition and food services, recordkeeping, hazardous substances, reporting injuries, emergency plans and transportation.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on attendance record review,the provider shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care.
Evidence:
1. The provider had a written hard copy of daily attendance without the arrival and departure times for the week.

Plan of Correction: Starting Monday will start to keep track of arrival and departure times.

Standard #: 22VAC40-665-160-D
Description: Based on children record review, the provider did not keep a child record and make accessible to department's representative for five years after termination of services or separation.
Evidence:
1. The provider did not have a record for Child #3.

Plan of Correction: I will keep in the future.

Standard #: 22VAC40-665-400-B-5
Description: Based on review of the emergency plan, the provider did not have lockdown procedures.
Evidence: The emergency plan dated 8/14/2016 did not include lockdown procedures.

Plan of Correction: I will add procedures.
