Comments:
An unannounced renewal inspection was conducted today from 10:15am-11:30am. There were 3 children (7 points) directly supervised by 1 staff. The home, 1 staff record, 2 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed watching television, playing freely and playing in a play kitchen. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based on record review, the provider did not review the emergency plan atleast annually.

Evidence: The emergency plan was last updated in 10/2016.

Plan of Correction: I will review the plan and update it if needed.

Standard #: 22VAC40-111-210-A
Description: Based on record review, all staff did not obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities.

Evidence:
1) Staff 1 does not have documentation for training hours completed in the 2018 year.
2) Staff 2 has documentation for 12 hours of the required 16 hours annual training.

Plan of Correction: We have started our hours for the 2019 year. We will complete the required 16 hours.
