63.2 Licensure and Registration Procedures

Technical Assistance:
Reminded the provider to space all occupied cribs at least 12 inches apart.

The license cannot be processed for renewal until the provider submits all documentation for each required background check as cited and discussed.

Comments:
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 10 children, totaling 26 points. Children's records, caregiver/household member records, background checks, the physical plant, children's nutrition, emergency preparedness items, and child care programming were inspected today. The provider reported that no children's medications were on site today.

The children were observed playing with play doh, engaging in gross motor skills indoor play, enjoying creative play activities, eating lunch, and watching a sign language video.
The children were served chicken nuggets, rice, apple slices, cooked carrots and milk for lunch today.

Each caregiver was observed patiently and calmly interacting with children who were displaying behaviors inconsistent with the programming.

Violations:
Standard #: 22VAC40-111-750-A
Description: Based on a review of children's topical ointment products, it was determined two items were on site without meeting all requirements.
Evidence:
Two containers of children's diaper ointment products were not labeled with each child's name.

Plan of Correction: They will be labeled today.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that four records do not contain all required information.
Evidence:
Child record A - D do not include documentation showing that each parent has reviewed their child's contact information within the past year.

Plan of Correction: Each parent will be asked to review their child's file.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on a review of background checks, it was determined that one required background check for a household family member has not been completed.
Evidence:
Household member C has come of age that a Central Registry background search is now required. The provider has not submitted the search request for this family member.

Plan of Correction: I will make sure the CPS search request form is completed and mailed in this week. I will send the documentation to my Inspector within 10 days as required.

Standard #: 63.2-1720.1-B-3
Description: Based on a review of background checks, it was determined that the provider has not submitted two out of state request searches of the Central Registry system.
Evidence:
One assistant caregiver indicated that she has resided outside of the state of VA within the past 5 years. The provider has not submitted out of state search requests of the Central Registry for each of the states in which she resided.

Plan of Correction: I will send both search requests this week and send documentation of each request to my Inspector within 10 days as required.
