Comments:
An unannounced renewal inspection was conducted on Monday, September 30, 2019 from approximately 10:15 AM to 12:20 PM to determine the provider's compliance with licensing standards. In attendance were 10 non-resident children. The children were being supervised by two caregivers during the inspection. There was a total of 26 points based on the children?s ages. The children were observed playing outside, playing with toys, looking at books, washing their hands and eating lunch. Lunch consisted of chicken nuggets, macaroni and cheese, green beans, pineapples and milk. The caregivers were observed engaging the children in conversation and providing assistance when needed. A walk through of the home was conducted with the provider. The child care areas were filled with supplies and toys for the children.

During the inspection, the licensing inspector reviewed the following: emergency supplies including first aid kit, battery operated weather radio, emergency plan, emergency drill logs, required postings, menu, caregiver's training hours, fireplace inspection and daily attendance. The caregiver has a current Medication Administration Training certificate. Transportation is not provided. During the inspection five children?s, four caregivers? and two household members? records were reviewed.

Standard #: 22VAC40-111-100-A
Description: Based on review of five children's records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
The record for Child #3 (start date 08/12/2019) contained documentation of a physical exam dated 09/27/2019.

Plan of Correction: The parent gave me the paperwork when she first enrolled but it could not be located during the inspection. I will make sure all paperwork is complete when it is submitted and filed in the record.
