63.2 Licensure and Registration Procedures

Technical Assistance:
Discussed placing a gate to divide infant area from play area.
Explained that caregiver needs fingerprint results prior to first day of employment.

Comments:
An unannounced renewal inspection was conducted today between the hours of 8:45 am through 10:30. There were a total of 8 children (21 points) observed with 3 caregivers. A total of 5 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, snack, bathroom breaks, and scarecrow art. Violations:
Standard #: 22VAC40-111-120-B
Description: Based on review of caregiver records, the record does not contain documentation of two or more references.
Evidence: Caregiver #1, hired 9/16/19 does not have documentation of two references, one reference is in the record.

Plan of Correction: I will get another reference for the caregiver.

Standard #: 22VAC40-111-200-A
Description: Based on observation and review of caregiver record, the provider has not provided orientation by the end of their first week of assuming job responsibilities.
Evidence: Caregiver #1, date of hire 9/16/19 does not have documentation of orientation by the end of their first week of assuming job responsibility. The caregiver was observed supervising children this day.

Plan of Correction: I will provider orientation.

Standard #: 22VAC40-111-210-A
Description: Based on interview and review of caregiver record, caregivers have not obtained 16 clock hours of training annually.
Evidence: Caregiver #2, date of hire 7/2016 has not obtained 16 hours of annual training. The caregiver stated she is taking the 10 hour subsidy training but has not completed the training.

Plan of Correction: I will take some more trainings.

Standard #: 22VAC40-111-500-C
Description: Based on observation, stationary equipment outside is installed over concrete.
Evidence: A Little Tykes climber, 24 inches high was observed in the outside garage sitting on concrete.

Plan of Correction: I have rubber tiles I will place under the slide.

Standard #: 63.2-1720.1-B-2
Description: Based on observation and review of caregiver record, caregiver has not submitted to fingerprinting prior to employment.
Evidence: Caregiver #1, date of hire 9/16/19 has not submitted fingerprinting prior to employment. Caregiver was observed providing care to the children this date with no fingerprint results in record.

Plan of Correction: I will send the caregiver to get fingerprinted.
