Technical Assistance:

Reviewed the Standard indicating that infant walkers are not to be used. There were no walkers in use during this inspection, however there was a walker available for use in the home.

Comments:

An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 7 children, totaling 20 points. Children's records, background checks, caregiver training, the indoor/outdoor physical plant, children's nutrition, caregiver:child interactions, and emergency preparedness items were inspected today. The children were observed playing with toys, practicing shape recognition skills, preparing for lunch, and interacting with each caregiver. The children were also scheduled to make cupcakes later today for a birthday celebration. The children were to be served fish sticks, peas, watermelon, and milk for lunch today. This inspection was completed between approximately 10:00am and 11:20am. Thank you for meeting with me today. Violations:

Standard #:	22VAC40-111-80-A
Description:	Based on a review of children's records, it was determined that the provider did not obtain one child's verification of identification within 7 business days of enrollment. Evidence: Child C entered care on 9/10/19. As of the date of this inspection on 10/2/19 the provider has not viewed the child's original verification of age and identity.
Plan of Correction:	The parent is still waiting to receive the child's birth certificate. I will ask her to bring the proof of birth letter.
Standard #:	22VAC40-111-480-D
Description:	Based on observations, it was determined that one piece of equipment was not being used in accordance with the manufacturer's instructions. Evidence: Upon arrival the Inspector observed a 2month old infant seated in a floor level portable high chair seat. This seat is designed and manufactured to be used with a safety buckle. The provider did not have the safety buckle attached to this piece of equipment. Because there was no safety buckle for this seat, the provider was not using it in accordance with the manufacturer's instructions.
Plan of Correction:	I will buy a safety buckle for the seat. I won't use for the children until I have the buckle.
Standard #:	22VAC40-111-530-A
Description:	Based on observations and provider interviews, it was determined that for nap, the children do not have linens consisting of a top and bottom cover or a one-piece covering that is open on three edges. Evidence: The older children who nap on rest mats do not have a top cover. These children are only given a bottom sheet that covers their rest mat.
Plan of Correction:	I wasn't aware that they had to have a blanket or top cover. I will make sure we start using either blankets or top sheets during nap.
