Comments:
An unannounced renewal monitoring inspection was conducted from 10:30am to 11:30am. At the time of inspector's arrival there were 12 children, ages 14mos to 3 years, for a total of 26 points, in care with the provider and an assistant. The records sample size consisted of 5 children's records and 2 provider / assistant records. Children were observed participating in indoor play and educational instruction. The provider stated that currently no medication is administered to children while in her care. The provider also reported there were no injuries since the last inspection. Areas of noncompliance are identified on the violation notice and discussed with the provider at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on record review and interview it was determined the provider did not ensure that each child's record contained all of the required components. Evidence: 1. The record for child #4(enrolled 2/21/18) did not contain the medical insurance name and policy number. 2. The provider confirmed that child #4's record did not contain the required medical insurance information.

Plan of Correction: Provider will request insurance information from the parent at the time of pick up.

Standard #: 22VAC40-111-240-A
Description: Based on observation it was determined the provider did not ensure that all areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition. Evidence: In the day care play room there were two 4' tall shelves of cubes that rocked when pulled on presenting a potential tip/crush hazard.

Plan of Correction: Provider will replace the anchors on both of the units to ensure they are stable and will not tip.
