Comments:
An unannounced renewal monitoring inspection was conducted from 12:02pm to 1:10pm. At the time of inspector's arrival there were 6 children, ages 18 months to 3 years, for a total of 11 points, in care with the provider and assistant. The records sample size consisted of 5 children's records and 3 provider / assistant/ household member records. Children were observed participating in departure, nap and interacting with the providers. The provider stated that currently no medication is administered to children while in her care. The provider also reported there were no incident reports since the last inspection. Areas of noncompliance are identified on the violation notice and discussed with the provider at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on record review and interview it was determined the provider did not review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct. Evidence: 1. The record for child #2(enrolled 8/5/18) did not contain an annual update. The documentation present was dated 8/5/18. 2. The provider confirmed that the record had not been updated since 8/5/18.

Plan of Correction: Provider will request this documentation from the parents at pick up.

Standard #: 22VAC40-111-90-C
Description: Based on record review and interview it was determined the provider did not obtain documentation of additional immunizations once every six months for children under the age of two. Evidence: 1. The record for child #2 did not contain documentation of immunizations once every six months until the child was two years of age. The last immunization documentation was dated 9/8/17. 2. The record for child #3 did not contain documentation of immunizations once every six months until the child was two years of age. The last immunization documentation was dated 1/29/19. 3. The provider confirmed that the record for child #2 and child #3 did not contain updated immunization documentation.

Plan of Correction: Provider will request this documentation from the parents at pick up.
